Report a Product

At the Artificial Intelligence Safety Forum (AISF), we work to make products that use generative AI safer for everyone. This form is only for reporting safety issues with these products – such as harmful responses, unsafe advice, or unmoderated content. It is not for technical support, feature requests, or product feedback.

Your report helps us identify risks, notify developers and platforms, and protect the public. Reports are reviewed confidentially, and you may submit anonymously or provide contact details if you’d like us to follow up.

We are also preparing a public incident database to raise awareness of risks and support transparency across the generative AI ecosystem. You’ll be able to opt in for your report to be included (with identifying details removed) once the database goes live (launch date TBC).

Together, we can improve AI safety and build more trustworthy technology.
